After Unlocking takes me to phone screen

When I unlock the phone it takes me to a screen I have never seen before that instead of my home screen. There are no numbers on the screen but has two rows with buttons on the top row from left to right it has an add call button, an end button, and a dialpad button. On the lower row it has a bluetooth button, a mute button, and a speaker button. What can I do to fix this so that it goes to my home screen?

I think that happens when your home or home replacement app crashes and it can't take you to that app, so it takes you to the uninitialized phone app instead. If you hit the home button when you get the funky phone screen, does it then take you to your normal 'home'?
