Contacts are getting all mixed up and intermingled with each other

I'm starting to have a reoccurring issue every 1-2 weeks now on my DX. My contacts are starting to get all mixed up with each other. For example, if I had a John Doe contact and a Paul Smith contact, over time the numbers for John Doe would show up under Paul Smith and vice versa. It's getting really frustrating. I have to go under Contacts in Application Manager and delete the content and then re-synch. It seems to happen very randomly. I'll search a contact and it shows someone else's name with their number along w/ the new contact number. It's all over the map and I need to find a permanent resolution here.
