Control Volume by Application?

I've been searching for an app which would allow me to control the volume for each application, no luck so far.

Here's an example situation... when I use the Google Maps Navigation, I set the media volume to the highest level because I'm in the car, it can be noisy and hard to hear the navigational voice prompts unless it's up full blast. Later when I return home, I open up a game to play and the media volume is set at the highest level, and it's too loud, so I turn down the media volume to the lowest level before mute.

I'd like the volume to be set at high each time I open Google Maps Navigation app, and the volume to be set at low each time I open other apps, like a game or music app, etc.

It's a nit, I know. It's not hard to turn up/down the volume when I switch between the applications, but it seems like there would be others who have that issue, and an app out there to manage it. I hope Any help is appreciated!!

This is Perfect app! Does exactly what I wanted it to do. It's simple, customizable enough, and the ads are unobtrusive. I use this every day to mute my phone at work with exactly the settings I want. Thanks!
