Is there not a way to add a direct dial icon to the desktop in MIUI?

yeah but it force closes as of right now but the way you would do it is:

menu > edit mode > menu > add > shortcut > direct dial

maybe this has been fixed on this weekend's release, well see

talked to the developer (IRC) as he flashed the next version and tried it. The issue will be solved in 12.17. Also the protected apps issue will be fixed.

You can use the favorite widget and touch the contacts name to direct dial
