Generic icon and app "disappears"?

What's up with picking up my phone after not using it for several hours, unlocking the screen, and seeing that random apps' icons have changed to a generic "monitor with a gear" icon, the app is not listed in the app drawer, but attempting to start the program with the odd icon works? What causes this?

Rebooting fixes is but there's no clear way to fix it interactively. I used to see this in Windows 95 and Windows 98 and sometimes Windows XP but to see the same thing in Android is annoying. Must be some corrupted icon caching mechanism but that doesn't explain the app missing from the app drawer.

The apps of note are not installed to SD card as I still have loads of space in the on-board 8GB.

Not a big deal but annoying nonetheless.
 
I have seen something similar but my icons actually disappear and calendar widget says jan 1. I realized it was cause of my live wallpaper. Its a mem/cpu hog but I like my live wallpaper.
