Need to go back to stock 4.0.4 but I can't connect via usb

Hi All,
I've somehow screwed up my phone so it can't connect via usb. I have a rooted 4.0.4 with some missing bloatware so the ota update to JB won't install. I'm using the stock cable, debugging is enabled and and it set to mtp. When I use Matt's 1.82, it just sits waiting for the device. I've tried to force the update from my sd card but that fails as well. Back to the usb issue. I don't get any notifications. Is here some process that might not running? Anyone have any ideas? I'd appreciate some help. I've been stuck on this for a while.

Are you using a front USB port on the computer tower? You need to use a backside dedicated USB port. I had the same problem you listed when using a front side port. I removed my external harddrive and plugged my phone into that dedicated port, installed the device driver and the utility worked fine.
 
I'm using a laptop and have tried all the usb ports. At one point it was working fine. But I think I might have disturbed a system file when I restored from Titanium.
