Rooted, S-ON, CleanROM installed, must return to Assurion

I shattered my screen and made a claim on my insurance (through Assurion), so I have to return my old phone. It's unlocked (via the HTC tool), rooted, S-on, and running Amon Ra recovery and CleanRom 4.5.

My questions are: Does Assurion give a damn if I rooted it? Can I just return it to them as is? And if not, how do I get it back to stock w/o using all the procedures I've ssen which require S-off? The reason I ask is I tried to do S-off when I rooted it, but because my eyes are not improving with age, I was unable to successfully execute the pin procedure.

Not sure about assurion, but when I sent my first Rezound back to Verizon, I relocked the bootloader, ran the official ics RUU, sent it back, and had no problems.
