SDcard Damage

I got the phone 4 days ago and my sdcard has been damaged 4 times. I've only put music and apk files on my sdcard....i have also rooted my phone. I'm getting tired of this, can anyone help me out with this...i've formated through the phone twice and 3rd and 4th ive taken it out and formated through the computer and yet the same results.

Yup, get them to exchange it.....wouldn't mention root tho....Call 611
