Call from contact showing up as unknown

I just received a voicemail from a number in my phone's address book, but when the call popped up on my screen, it showed up as an unknown call with no phone number, so I dismissed it.

Why would a call from a number in my address book come through as an unknown number?

that is most likely a network issue, not your phone, just was lost in transmission

That could be, except it's the opposite of what happened to me. My work called, but it came through as a blocked number instead of their number. I only knew it was my work after they left a voice mail.
