new contact fields missing

So I've searched for a while on this and im guessing its a bug. I have a user on moto droid who goes into contacts and tries to enter a new one and only first and last name display? No other fields like phone number etc. She's on 2.2 and as far as I know hasn't messed with any settings. Anyone see this issue? Even if we edit a new contact we still can't see any other fields but first/last name? Something easy im overlooking? Any help is appreciated thanks!
