phone options pop up constantly after removing google voice

I had Google voice and couldn't stop the service. I used a code to officially get my calls routed back to my number after uninstalling the app. It worked. I got rid of GV but my phone optons menu keeps popping up (airplane mode, shut down, silence). I've done a hard reset and wiped everything off the phone am but still having the pop up problem. My phone is running slower now too. I have a stock Motorola Droid. Please let me know if you have any suggestions.

P.S. I hate Google voice! There was nothing wrong with my phone until this. Beware.
