I have a Galaxy Nexus on Verizon and have a Verizon data plan that includes wifi hotspot (tethering).
The phone seems to have broken tethering. Here's what I've tried:
Needless to say, a laptop cannot get on the Internet with it in the "Error" state.

- Settings -> Wireless & Networks area -> More
- Tethering & portable hotspot
- Check Portable WiFi hotspot
- First, "Checking your subscription"
- Then "Turning wifi on"
- Then "Portable hotspot testing active" (testing is my ssid -- I've tried several)
- Then about 0.5 to 3 seconds later (it varies a bit), just "Error". The green checkbox is still there, but it says "Error" under the menu item.

- Change SSID
- Change password
- Turn WiFi on and off, and try it from those states
- Power cycle the phone
- Called Verizon for help
- They had me try a bunch of things over the course of an hour but wound up baffled.
