Wifi Tethering Broken

I have a Galaxy Nexus on Verizon and have a Verizon data plan that includes wifi hotspot (tethering).

The phone seems to have broken tethering. Here's what I've tried:

  • Settings -> Wireless & Networks area -> More
  • Tethering & portable hotspot
  • Check Portable WiFi hotspot
And here's what it does:
  • First, "Checking your subscription"
  • Then "Turning wifi on"
  • Then "Portable hotspot testing active" (testing is my ssid -- I've tried several)
  • Then about 0.5 to 3 seconds later (it varies a bit), just "Error". The green checkbox is still there, but it says "Error" under the menu item.
I wish there was a more descriptive error, but there just wasn't on the UI. So I installed aLogCat. It displayed, among other things:

Needless to say, a laptop cannot get on the Internet with it in the "Error" state.

To attempt to solve it, I tried, in this order:
  1. Change SSID
  2. Change password
  3. Turn WiFi on and off, and try it from those states
  4. Power cycle the phone
  5. Called Verizon for help
    1. They had me try a bunch of things over the course of an hour but wound up baffled.
Any other ideas?

I'm using this, and for some reason it only works for about 30 minutes and then stops.
The app looks like it's running, but my PC doesn't see it.

I had used it on my OG Droid and had no problems.
I am NOT running any tasker or battery saver type programs that could conflict with the settings.

Any thoughts on how to fix this?

Today I got it to go for about 5 hours before it dropped the connection.

Could it be that my connection drops when the phone attempts to go to 1x?
Or could it be some other fluke like that?

Do we suppose a future update (radio, 4.0.3, etc) will fix it?
