what theme is that? I like it a lot

It is y0rks orange adw theme.

Is that Clock and Date part of that theme from adw or is it a different app?

Clock: Weather Widget (don't think it's available exactly like this anymore, try Fancy Widget)
Calendar: Jorte
Battery/Storage: Mini Info Widget
