I believe those were made through Minimalistic Text.

Haven't uploaded anything with the app in a long time.

Nah man I hardly upload from my phone anymore. I guess I could now that I don't have to edit the image before uploading seeing that I keep my calendar on the lockscreen these days. But it used to work when I did use it. However, sometimes the uploaded image didn't select the best server and was a little on the pixelated side.

This is a screenshot of my Samsung Fascinate. I like the clean look. All of my widgets and visible shortcuts are on the other four screens. I have all of the typical shortcuts on the dock, I just made them invisible. Left to right: Phone Dialer, Contacts, Droid eye is to open drawer, then Handcent, and browser.
I am going to make the black background of the Droid eye transparent. That will finish the look I think. I found the pic online and liked it a lot! Thank you to whoever created it, I give you all of the credit!
