Took the Plunge!
Well I took the plunge today and applied the 4.0.4 update. I had rooted an OG Droid a couple years ago so I had a little clue. After going to multiple websites to relearn, I now have 4.0.4!!! I actually just recently switched to a Rezound because I was so tired of my Nexus failing me but had issues with the CLNR Rezound after day 2. I heard that the update was coming out for Nexus and today decided I was going back to my Nexus, which I hadn't sent back yet, and installing the update myself. I absolutely love the Nexus and didn't want to part with it so I'm glad I was able to apply the latest update.

Yeah, I used CWM Touch. Follow this link and download the Nexus toolkit. It makes it super easy. [GNex TOOLKIT V6.2] Drivers, Backup, Unlock, Root, CWM, Flash, Mods + MUCH MORE [LTE]

From his description I assumed he flashed in CWM. You can flash in stock recovery, but it has to be in /cache. If that's what you're doing, I have no idea why it's not working. I'd just fastboot a cwm img real quick and then flash the zip in the custom recovery (this is assuming your bootloader is unlocked though)

Did you grant R/W access to the /cache folder? It's Read only by default.

Woo hoo! Glad it was something simple like permissions. Lol. Enjoy the new update. Been running it for several weeks now.

So for those of us who are not rooted and are not unlocked, I am guessing that the link to this file will not let us upgrade to 4.0.4. Right?
I am not familiar with how to move files to cache and provide permissions, but I would really like to upgrade. Are there some easy instructions to do this?

If you aren't already unlocked, I wouldn't suggest doing so unless you REALLY want to upgrade. It wipes your internal storage when you unlock. If you still want to do it, I'd be happy to help. Just want you to understand what will happen.

Got it. Thanks for your offer, but in that case I think I will just have to wait. It is really frustrating to have to wait for this. If you blame Verizon, they blame Google and/or Samsung and vice versa.
There is a way to flash it without rooting, though I believe it temporarily roots/unlocks your phone, then reverts it back to stock:

At this rate sometime near Armageddon
