Lockscreen:
widgetlocker with the honeycomb slider
clock/weather/battery widget is a UCCW skin called "cute clock weather free" and as the title says its free in the play store.

homescreen:
using nova launcher prime changed dock to 7 icon dock icons shrunk down to 75% and removed the clock on the homescreen but shows on lockscreen and when apps are open (done within nova settings)
icons are "SIML White" i know there in the play store but dont remeber if they are free or not.
clock widget is a UCCW skin called "Elegante clock" and its free in the play store.
