Last week I got a TB off ebay - walked into verizon store - they gave me one free; activated the phone, and deactivated my last phone, and said have a great day...
They didn't even ask where the phone was from - as long as the ESN is good.

Lol i bought my tb off of craigslist and got my sim card for free! First ones are free! After that they said I would have to pay if I lose that one, so just go to verizon, your guna buy one on ebay when you can get it for free! With 6 dollars you can buy your self a nice case for it ) well at the swapmeet only... Lol

1) I'm doing the same thing. Will have to go to Verizon, get LTE SIM card, have them activate it.
2) I'm not scared to go to a physical store and interact with real people.
3) I'm guessing the Thunderbolt wasn't less than $100. I think you can afford to pay $10, if that, to get a replacement SIM from Verizon. ... It's just like when people b*tch about a $0.99 app after buying the phone for $200. Duh.
