Clock is Simi Clock. The dock is transparent, but the icons are HOOGE 84X by yuushaART (see below link).

I did a quick Google search but couldn't find anything. Don't have time for an extensive one but I know Matt would have the link. I'm almost positive he's the one that posted it.
